Pre-Surgery Checklist
If you are coming to Beverly Hills Medical Group for
treatment, please make sure of the following:
Passport
Please make sure that your passport is valid and
that it does not expire within 6 months of your
arrival in the Philippines.
Visa
Visitors are allowed entry without a visa provided
that their stay does not exceed 31 days, they have a
passport that is valid for at least 6 months beyond
the period of their intended stay, and that they
have a return ticket or a ticket to another
destination outside the Philippines. For visitors
who intend to stay for more than 31 days, a
visitor’s visa mU.S.t be secured from the Philippine
Embassy or consulate in their respective country.
Medical
History
As part of your consultant you will be required to
give your surgeon your medical history. (see
more details about consultations) Please
be sure to bring these details with you.
